A Blood Death Knight running San'layn found /cast Heart Strike had been rewritten to /cast Vampiric Strike, which is not a name the game will cast. A Guardian Druid found /cast Maul and /cast Ravage doing nothing at all. Demon Hunters and Monks saw Cull, Eradicate and Rushing Wind Kick left sitting in the step list. It is all one cause. The addon kept your step text in step with your talents by asking the game which spell is active right now and saving the answer, and the game answers that question the same way for a talent you chose as for a form you are standing in, a channel you are part way through, or a buff that happens to be up. A name that only exists for a few seconds went into the sequence and stayed there. The previous release skipped that rewrite during combat, which was not enough on its own: a proc can still be running the moment you leave combat, and leaving combat is exactly when the rewrite was set to run again. The swapped name is not written anywhere now. Your steps keep the ordinary base name, which is the name the game wants, since casting Maul casts whatever Maul currently turns into. The swapped name is worked out fresh each time it needs to be shown, where getting it wrong costs a label instead of a macro that does nothing.

3Because the swapped name is no longer saved, the step list works it out as it draws each row instead: a step written as Maul reads as Ravage while Ravage is the version your talents give you, and reads as Maul again on its own once it is not. That second half needed the same short list of confirmed pairs the repair uses, because for some spells the game does not report what the swapped name stands in for, and without it a step could keep reading Ravage after you had untalented Ravage. What it cast was right the whole time; only the label was stale. A row you are part way through editing shows what you typed rather than the worked-out name, so nothing paints over you mid-edit. Getting this wrong now shows the wrong label for a moment and corrects itself; it can no longer leave a macro that does nothing.
The repair runs once, the first time the addon reads your spellbook after you update, and puts the ordinary spell name back into any step, keypress or keyrelease it had been written over. Some steps have the swapped spell itself saved underneath, not just its name, and those need a second look. Where the swapped spell still records which spell it stands in for, the original comes back too, so a step saved as Raze returns to Maul. Where it records nothing, the addon checks a short list of pairs that have been confirmed on a live character, which is how a step saved as Ravage also returns to Maul. Only a spell that fails both checks is left alone, exactly as it is, instead of being filled in with a guess. You can retype those, and the list grows as more pairs are confirmed.
